About Our Team

We are a dynamic and innovative team at the Royal Devon University Healthcare NHS Foundation Trust, dedicated to providing high-quality patient care and improving health outcomes. As a Care Pathway Manager, you will play a vital role in coordinating care and ensuring seamless transitions between different healthcare settings.

Your clinical expertise and leadership skills will enable you to develop effective care plans and lead the team in delivering high-quality patient care. You will work closely with healthcare professionals to ensure that patients receive coordinated and person-centred care, reducing unnecessary hospital admissions and optimizing patient flow.

The successful candidate will be expected to use advanced clinical assessment skills to identify opportunities for care coordination and collaboration with other healthcare professionals. You will liaise with patients and carers to discuss discharge planning and provide training and support to develop the team's skills.

Main Responsibilities
  • To provide and review nursing care, advice, and information to patients and carers within the home environment.
  • To maintain patients in their preferred place of care, enabling them to maximize their independence and optimize their quality of life.
  • To collaborate with the multi-disciplinary team to avoid unnecessary hospital admissions and focus on patient flow.
Requirements
  • Clinical skills required include advanced clinical assessment, catheterization, venepuncture, wound care, end-of-life care, IV therapies, and assessing and ordering equipment.
  • Flexible working hours between 8:00 - 20:00, 7 days a week.
What We Offer

We offer a competitive salary range of £38,000 - £50,000 per annum, depending on experience, and a range of benefits to support your well-being and career development.
