Senior Care Coordinator Role

About the Role: We are seeking an experienced Senior Care Coordinator to join our team in Exeter and East Devon. The successful candidate will be responsible for coordinating care services, ensuring high-quality support for individuals with learning disabilities.

Responsibilities
  1. Co-ordinate daily care activities, working closely with the support staff.
  2. Develop and implement individualized care plans, focusing on promoting independence.
  3. Conduct thorough risk assessments to safeguard adults with learning disabilities.
  4. Maintain accurate records of client progress and communicate effectively with colleagues.
  5. Liaise with external agencies, building strong relationships to enhance service delivery.
Qualifications and Experience
  • Minimum NVQ Level 3 in Social Care or equivalent qualification.
  • Proven experience in supporting individuals with learning disabilities.
  • Strong organizational and time management skills.
  • Excellent communication and interpersonal skills.
