Clinical Lead Nurse Specialist

We are seeking an experienced Clinical Lead Nurse Specialist to join our team at the Royal Devon University Healthcare NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality nursing care and advice to patients and their carers within the home environment.
With a strong focus on patient-centred care, you will work collaboratively with healthcare professionals to avoid unnecessary hospital admissions and optimize patient flow. Your clinical assessment skills will enable you to identify opportunities for care coordination and develop effective care plans.
The successful candidate will be expected to use advanced clinical assessment skills to identify opportunities for care coordination and collaboration with other healthcare professionals. You will liaise with patients and carers to discuss discharge planning and provide training and support to develop the team's skills.
Main Responsibilities- To provide and review nursing care, advice, and information to patients and carers within the home environment.
- To maintain patients in their preferred place of care, enabling them to maximize their independence and optimize their quality of life.
- To collaborate with the multi-disciplinary team to avoid unnecessary hospital admissions and focus on patient flow.
- Clinical skills required include advanced clinical assessment, catheterization, venepuncture, wound care, end-of-life care, IV therapies, and assessing and ordering equipment.
- Flexible working hours between 8:00 - 20:00, 7 days a week.
We offer a competitive salary range of £38,000 - £50,000 per annum, depending on experience, and a range of benefits to support your well-being and career development.
