Emergency Department Mental Health Professional

We are seeking a skilled Mental Health Liaison Nurse to join our team at the Queen Elizabeth Hospital. As a Band 6 nurse, you will be responsible for supporting patients in the emergency department and on inpatient wards, working closely with multidisciplinary teams to ensure seamless care transitions.
The ideal candidate will have a strong understanding of mental health issues, particularly those affecting older adults. You will conduct thorough biopsychosocial assessments, formulate effective care plans, and collaborate with various stakeholders to ensure patient safety and well-being.
Role Overview
This exciting opportunity is for a Mental Health Liaison Nurse to join our older adults mental health liaison team. You will provide vital support to patients in the emergency department and on inpatient wards, working closely with multidisciplinary teams to ensure seamless care transitions.
As a key member of our team, you will be responsible for conducting thorough biopsychosocial assessments, formulating effective care plans, and collaborating with various stakeholders to ensure patient safety and well-being.
Key Responsibilities
* Conduct timely and effective biopsychosocial assessments of individuals referred to the team from the emergency department and inpatient wards
* Formulate and implement appropriate care plans, considering all necessary sources of information
* Work collaboratively with multidisciplinary teams to carry out risk assessments and management, ensuring patient safety and well-being
Salary: £38,890 - £44,884 per annum (dependent on experience)
