About the Opportunity

We are seeking an experienced and compassionate Community Support Practitioner to join our Adults Locality Team in Medway, Kent.

Paying up to £38 per hour

  • Needs Analysis: Conduct thorough analyses of adults' needs, taking into account physical, psychological, and social factors. Develop personalized care plans in collaboration with service users, their families, and other professionals, ensuring that interventions promote independence and meet individual goals.
  • Caseworker: Manage a diverse caseload of adults with varying needs, including older adults, individuals with disabilities, and those with mental health concerns. Provide ongoing support, advocacy, and coordination of services to ensure that individuals receive the appropriate care and support.
  • Vulnerable Adult Protection: Identify and respond promptly to safeguarding concerns, adhering to local policies and procedures. Collaborate with safeguarding partners, such as health professionals and law enforcement agencies, to ensure the safety and well-being of vulnerable adults.
  • Collaborative Teamwork: Work effectively with colleagues from various disciplines, including health professionals, housing officers, and voluntary organizations, to provide holistic support to service users. Participate in collaborative meetings and case conferences to coordinate care and facilitate joint working practices.
  • Independence Promotion: Support adults to maintain or regain their independence by accessing community resources, social activities, and housing options. Provide practical assistance with tasks such as budgeting, housing applications, and accessing benefits to promote self-sufficiency and empowerment.
Required Qualifications and Skills
  • A qualified social worker with 2 years plus experience in adult's social care.
  • A car driver.
  • Social Work England registered.

Estimated Salary: £38 per hour
