Lead District Nurse in a Thriving Community Setting

We are seeking an experienced and passionate District Nursing Team Leader to join our team at West London NHS Trust, leading the way in delivering high-quality, integrated nursing care to patients within the Acton community.

This is an exceptional opportunity for a dedicated clinical leader who is committed to enhancing community health through the delivery of innovative, patient-centric care. You will be responsible for managing and supporting a skilled team of nurses, with a focus on preventing unnecessary hospital admissions and promoting patient independence.

As a key member of our multidisciplinary team, you will work closely with GPs, District Nurses, Social Workers, Rapid response teams, Palliative care teams, and other healthcare professionals to provide holistic, patient-centered care within the Acton community.

To succeed in this role, you will require strong organizational and leadership abilities, with experience in management and education. You should be able to foster a positive, educational environment that supports staff development and upholds the highest standards of care in line with our values of Togetherness, Responsibility, Excellence, and Caring.

Salary: £53,160 - £69,919 per annum
