About the Role

We are seeking a highly motivated and compassionate individual to join our SMILE team as a SMILE Outreach Worker. As a key member of our team, you will play a vital role in supporting adults with learning disabilities, physical disabilities, autism, older people, and individuals living with dementia to achieve their chosen outcomes and meet their goals.

As a SMILE Outreach Worker, you will be responsible for providing support with all aspects of daily living, including personal care. You will also have the opportunity to work with a diverse range of individuals, promoting their independence, dignity, and well-being.

Key Responsibilities
  • Provide support with daily living tasks, including personal care and mobility
  • Assist individuals with their chosen outcomes and goals
  • Work collaboratively with the SMILE team to promote independence and dignity
  • Contribute to the development and implementation of individual support plans
  • Participate in training and development opportunities to enhance your skills and knowledge
