About the Role

Creative Support is seeking a dedicated and compassionate Mental Health Housing Navigator to join our team. As a key member of our Homelessness & Housing Options Service, you will play a vital role in supporting individuals who are homeless or at risk of homelessness and have mental health needs.

Key Responsibilities
  • Work closely with the Homelessness & Housing Options Service and the NHS Access & Crisis Team to identify individuals who require specialist support.
  • Provide advice and guidance on housing and support options, including assistance with locating and resettling into independent tenancies.
  • Collaborate with a range of mental health professionals and housing providers to ensure comprehensive support for service users.
  • Develop and maintain relationships with local stakeholders to enhance service delivery and outcomes.
Requirements
  • Essential driving skills and a valid driver's license.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Strong problem-solving and crisis management skills.
