Creative Support is seeking a dedicated and compassionate Mental Health Housing Navigator to join our team. As a key member of our service, you will play a vital role in supporting individuals who are homeless or at risk of homelessness and also have mental health needs.
Key Responsibilities- Work closely with the Homelessness & Housing Options Service and the NHS Access & Crisis Team to identify individuals who need specialist support.
- Provide advice and guidance on housing and support options, including support to locate and resettle into an independent tenancy.
- Collaborate with a range of mental health professionals and housing providers to ensure individuals receive the best possible support.
- Develop and maintain relationships with local services and stakeholders to ensure a seamless service delivery.
