**About the Role**

Creative Support is a leading provider of person-centered social care services for individuals with mental health needs. We are seeking a highly motivated and compassionate individual to join our team as a Mental Health Housing Navigator.

**Key Responsibilities**

  • Work closely with the Homelessness & Housing Options Service and the NHS Access & Crisis Team to identify individuals who require specialist support.
  • Provide advice and guidance on housing and support options, including assistance with locating and resettling into independent tenancies.
  • Collaborate with a range of mental health professionals and housing providers to ensure seamless support for individuals.
  • Develop and maintain relationships with local stakeholders to enhance service delivery.

**Requirements**

  • Essential driving skills
  • Ability to work in a fast-paced environment
  • Strong communication and interpersonal skills
  • Knowledge of mental health services and housing options
