About the Role

Creative Support is seeking a dedicated and compassionate individual to join our team as a Mental Health Housing Navigator. This is a unique opportunity to make a positive impact on the lives of individuals experiencing homelessness and mental health needs.

Key Responsibilities

  • Work closely with the Homelessness & Housing Options Service and the NHS Access & Crisis Team to identify individuals who require specialist support.
  • Provide advice and guidance on housing and support options, including assistance with locating and resettling into independent tenancies.
  • Collaborate with a range of mental health professionals and housing providers to address housing-related issues.
  • Develop and maintain relationships with local service providers to ensure seamless support for clients.

Requirements

  • Essential driving skills
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
  • Knowledge of housing and mental health services
