About the Role

Creative Support is seeking a dedicated and compassionate Mental Health Housing Navigator to join our team. As a Mental Health Housing Navigator, you will play a vital role in supporting individuals who are homeless or at risk of homelessness and also have mental health needs.

Key Responsibilities
  • Work closely with the Homelessness & Housing Options Service and the NHS Access & Crisis Team to identify individuals who need specialist support.
  • Provide advice and guidance on housing and support options, including support to locate and resettle into an independent tenancy.
  • Collaborate with a range of mental health professionals and housing providers to address housing issues and provide holistic support.
  • Develop and maintain relationships with local services and stakeholders to ensure seamless support for service users.
