Job Title: NHS Continuing Healthcare Nurse Assessor

We are seeking a highly skilled and experienced Nurse Assessor to join our Personalised Healthcare Commissioning team in Staffordshire. As a key member of our team, you will play a vital role in supporting the delivery of high-quality healthcare services to our patients.

Job Overview

The successful candidate will be responsible for assessing patients to determine their eligibility for NHS Continuing Healthcare funding. You will work closely with our multidisciplinary team to ensure that patients receive the best possible care and support.

Main Duties and Responsibilities
  • Conduct comprehensive assessments of patients to determine their eligibility for NHS Continuing Healthcare funding.
  • Work collaboratively with our multidisciplinary team to ensure that patients receive high-quality care and support.
  • Manage a caseload of complex cases and provide specialist advice and guidance to colleagues.
  • Contribute to the development and implementation of care plans that meet the individual needs of patients.
  • Monitor and evaluate the effectiveness of care plans and make recommendations for improvement.
Person Specification

We are looking for a highly skilled and experienced Nurse Assessor who possesses the following qualities:

  • Experience of working in a healthcare environment.
  • Registered Nurse with a valid UK nursing registration.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Strong analytical and problem-solving skills.
