About the Role

We are seeking a highly skilled and experienced Nurse Assessor to join our team at NHS Midlands and Lancashire Commissioning Support Unit. As a Senior Practitioner, you will play a key role in supporting the Personalised Healthcare Commissioning service across the region.

Key Responsibilities
  • Assess patients to determine eligibility for NHS Continuing Healthcare and Funded Nursing Care, ensuring compliance with the National Framework.
  • Manage a caseload of complex cases and lead on the development of Personalised Healthcare packages of care.
  • Work collaboratively with multi-disciplinary teams to facilitate assessments and reviews.
  • Provide specialist advice and guidance on functional and information matters relating to the service.
  • Contribute to performance improvement within the team, ensuring services are delivered to a high standard.
Requirements
  • Experience of working in a health environment, preferably in a commissioning or assessment role.
  • Education to degree level in a relevant subject or equivalent level of qualification or experience.
  • Healthcare Professional Registration (RMN).
  • Extensive knowledge of specialist areas, including continuing healthcare and multi-disciplinary working.
  • Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information.
