Job Title: NHS Continuing Healthcare Nurse Assessor

We are seeking a highly skilled and experienced Nurse Assessor to join our Personalised Healthcare Commissioning team in Staffordshire. As a key member of our team, you will play a vital role in supporting the delivery of high-quality healthcare services to our patients.

About the Role

The successful candidate will be responsible for assessing patients to determine their eligibility for NHS Continuing Healthcare funding. You will work closely with our multidisciplinary team to ensure that patients receive the best possible care and support.

Key Responsibilities
  • Conduct comprehensive assessments of patients to determine their eligibility for NHS Continuing Healthcare funding
  • Work collaboratively with our multidisciplinary team to ensure that patients receive high-quality care and support
  • Manage a caseload of complex cases and provide specialist advice and guidance to colleagues
  • Contribute to the development and implementation of policies and procedures related to NHS Continuing Healthcare
  • Participate in quality assurance and performance improvement activities to ensure that our services meet the highest standards
Requirements

To be successful in this role, you will need to have:

  • Experience of working in a healthcare environment, preferably in a role related to continuing healthcare
  • Education to degree level in a relevant subject or equivalent level of qualification or experience
  • Healthcare Professional Registration (e.g. RMN)
  • Extensive knowledge of specialist areas acquired through experience and training, plus further specialist knowledge of the continuing healthcare process and MDT working
  • Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales
