Main Responsibilities- Assist in ensuring the efficient and effective admission and discharge planning process to maintain patient flow.
- Promote and maintain a safe and therapeutic environment for patients, respecting their views and beliefs, while awaiting discharge.
- Establish and maintain excellent communication with patients and their families/carers regarding discharge plans.
- Liaise with the multidisciplinary team to ensure a smooth discharge process.
- Participate in the assessment of care needs for patients on admission and discharge.
- Ensure the effective and efficient use of resources.
