Job Summary

Main Responsibilities
  • Assist in ensuring the efficient and effective admission and discharge planning process to maintain patient flow.
  • Promote and maintain a safe and therapeutic environment for patients, respecting their views and beliefs, while awaiting discharge.
  • Establish and maintain excellent communication with patients and their families/carers regarding discharge plans.
  • Liaise with the multidisciplinary team to ensure a smooth discharge process.
  • Participate in the assessment of care needs for patients on admission and discharge.
  • Ensure the effective and efficient use of resources.
