The post holder will be expected to organise their own workload and coordinate their activities in conjunction with the IPOCH practitioners and IPOCH team leaders. Be an active member in daily huddles with other members of the IPOCH team. Take a pivotal role in ensuring the systems and processes of admission and discharge are expedited efficiently and effectively to maintain patient flow. Promote and maintain a safe and therapeutic environment for patients, respecting their views and beliefs, whilst under our care.

Required to liaise with patients, families and members of the MDT establishing and maintaining excellent communication including with partners from outside of MCHFT. May be expected to attend multidisciplinary and/or family meetings. Participate in the assessment of care needs for the patient on admission and discharge and to commence plans by referring to the appropriate multi-disciplinary team, including completion and submission of STTF to community triage. Will be required to drive between sites to support completion of assessments in Pathway 2 settings.

Ensure the legibility and accuracy of referrals to other professionals via written, verbal, and digital formats, making sure all such referrals are completed in a timely manner. Confirming outpatient appointments, liaising with appropriate teams, or utilising trust-based web systems to confirm. Booking and coordinating transport, liaising with ambulance department, community connectors and voluntary agencies. Maintain accurate record keeping on systems used including EMIS and gateway to reflect current bed status.

Ensure all members of the multi-disciplinary team are aware of issues that may affect smooth admission and discharge for each patient. Assist in meeting trust objectives by ensuring patients are admitted and discharged into & out of service efficiently and appropriately. Monitoring of readmissions from pathway 2 beds, updating acute staff of previous plans thus ensuring good communication and handover between acute and community services. Review and amend IDD data under direction of IPOCH practitioners.

Maintain health, safety and security and report any concerns. Contribute to the ongoing development of the IPOCH service. Develop own skills and knowledge and support the development of others. Always maintain appearance and behaviour befitting a member of the IPOCH team.
