Clinical Discharge Coordinator

About the Role:
We are seeking an experienced Clinical Discharge Coordinator to join our team at Mid Cheshire Hospitals NHS Foundation Trust. The successful candidate will be responsible for coordinating the discharge process for complex stroke and medical patients, ensuring a safe and effective transition to the next stage of care.
Main Responsibilities:
- Collaborate with the multidisciplinary team to develop and implement discharge plans
- Communicate effectively with patients, relatives, and healthcare professionals to ensure a smooth transition of care
- Manage and coordinate the discharge process, ensuring timely and efficient transfer of patients
- Review and update patient information, maintaining accurate and comprehensive records
- Participate in the development and implementation of policies and procedures to improve discharge practices
- Contribute to the ongoing evaluation and improvement of discharge services
- Work collaboratively with other departments to ensure seamless care transitions
- Maintain up-to-date knowledge and skills, participating in training and professional development opportunities
- Comply with Trust policies, procedures, and standards, ensuring a safe and respectful work environment
- Participate in quality improvement initiatives, contributing to the advancement of discharge practices
Person Specification:
- Excellent communication and interpersonal skills
- Proven experience in discharge coordination or a related field
- Strong organizational and time management skills
- Ability to work effectively in a fast-paced environment
- Knowledge of healthcare policies, procedures, and standards
- Ability to maintain confidentiality and handle sensitive information
- Proficient in computer skills, including Microsoft Office and electronic patient records
- Previous experience working in a healthcare setting, preferably in a discharge coordination role
