Care and Support Practitioner

We are seeking dedicated Community Living Coordinators to join our team at Creative Support Careers. As a Community Living Coordinator, you will play a vital role in supporting individuals with autism and other needs to live fulfilling and valued lives.
About the Service:
Our bespoke supported living service in Little Thurrock provides an empowering, strengths-based model of support that enables individuals to develop their quality of life and independence. You will work closely with colleagues to promote person-centred support and best practice, ensuring that each individual receives tailored support to meet their unique needs.
Your Responsibilities:
* Provide care and support to individuals with autism and other needs, promoting independence and self-advocacy
* Act as a Key Worker, supporting individuals to achieve their dreams and goals
* Work collaboratively with colleagues to promote effective relationships with service users, families, and community professionals
* Develop and maintain person-centred plans that reflect individual needs and preferences
Requirements:
* Experience of working with vulnerable adults, particularly those with autism or learning disabilities
* Ability to work flexibly and respond to changing circumstances
* Strong communication and interpersonal skills
* Commitment to person-centred values and principles
Benefits:
