About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our team at Right at Home Croydon to Caterham. As a key member of our leadership team, you will be responsible for managing and owning our domiciliary care planning activity, ensuring that our clients receive high-quality care in the comfort of their own homes.

Key Responsibilities
  • Manage and own our domiciliary care planning activity, ensuring seamless delivery of care services to our clients.
  • Prepare and complete care schedules for our carers, utilizing Access People planner to optimize care delivery.
  • Build and develop care schedules with carers, minimizing travel time and maximizing efficiency.
  • Onboard new clients to our care schedules, matching them with suitable carers.
  • Develop and maintain strong working relationships with our care team, ensuring effective communication and collaboration.
  • Provide exceptional customer service to our clients, NOK, and healthcare professionals, addressing their care-related queries and concerns.
  • Ensure compliance with regulatory requirements within the office and care calls, maintaining accurate records and audit trails.
  • Support the registered manager in their duties, contributing to the overall success of our care service.
Requirements
  • 2+ years of experience in care management, with a focus on domiciliary care planning and coordination.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, carers, and healthcare professionals.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to work under pressure, maintaining a calm and professional demeanor in challenging situations.
  • Reliability, flexibility, and a commitment to delivering high-quality care services.
