We are seeking a highly skilled and experienced Care Coordinator to join our team at Affinity Care Recruitment. As a Care Coordinator, you will play a vital role in ensuring the smooth operation of our domiciliary care services.
Key Responsibilities- Produce care staff rotas on a weekly basis using our computer-based roster system.
- Maintain accurate and up-to-date records of care staff and client information, ensuring compliance with organisational and legal requirements.
- Review and allocate referrals with Field Care Supervisors, ensuring efficient use of resources.
- Compile client information and Home Care Worker management actions, reporting weekly on Key Performance Indicators (KPIs).
- Allocate calls to care workers, taking into account Service User needs and preferences.
- Ensure all calls are covered, providing care workers with full briefings on Service User needs and any other relevant information.
- Proactively liaise with Social Workers, Service Users, Family Members, and Care Workers to take on new referrals and arrange initial assessments.
- Communicate effectively with Service Users, staff, and other stakeholders to promote a high standard of care and support.
- Ensure confidentiality of Service User information, in line with our organisation's policy and procedures.
- Report any incidents or complaints to your line manager, completing incident report forms and other documentation as required.
- NVQ level 2 (or equivalent) in Health and Social Care.
- Experience working in a domiciliary care setting, with a strong understanding of CQC regulations and Health and Safety requirements.
- Excellent communication and problem-solving skills, with the ability to work independently and as part of a team.
- Proficiency in using computer packages such as Microsoft Office, Carefree, and EziTracker.
- A flexible approach to working hours, with the ability to work as part of a team and on your own initiative.
