We are seeking a highly skilled and experienced Care Coordinator to join our team at Affinity Care Recruitment. As a Care Coordinator, you will play a vital role in supporting our business by working closely with Local Authority Health care Teams and other providers.
Key Responsibilities- Produce care staff rotas on a weekly basis using our computer-based roster system.
- Maintain accurate and up-to-date records, ensuring compliance with organisational and legal requirements.
- Review and allocate referrals with Field Care Supervisors, ensuring efficient use of resources.
- Compile client information and Home Care Worker management actions, reporting weekly using KPI forms.
- Allocate calls to care workers, considering Service User needs and preferences.
- Ensure all calls are covered, providing care workers with full briefings and updates.
- Maintain accurate records on the Carefree roster, including Service User and staff information.
- Communicate effectively with Service Users, staff, and other stakeholders to promote a high standard of care.
- Ensure confidentiality of Service User information, in line with our policy and procedures.
- Report incidents or complaints to your line manager, completing incident report forms and other documentation as required.
- NVQ level 2 qualification, with willingness to work towards NVQ level 3 and supervision/management qualifications.
- Excellent communication and problem-solving skills, with experience in using a roster within Domiciliary Care.
- Ability to work flexibly, with a commitment to working as part of a team and on your own initiative.
- Competent in using computer packages such as Microsoft Office, Carefree, and other relevant applications.
- Good team ethic and working knowledge of regulatory frameworks (CQC) and Health and Safety requirements.
