We are seeking a highly skilled and compassionate Community Mental Health Nurse to join our team at Lancashire & South Cumbria NHS Foundation Trust. As a key member of our Care Home Liaison Service, you will play a vital role in providing person-centered care to individuals with complex mental health needs.
Key Responsibilities- Work closely with care home staff, OTs, nurses, doctors, and the wider MDT to maintain positive health and well-being for service users.
- Provide comprehensive written assessments and develop care plans to support the needs of individuals with complex mental health needs.
- Undertake ECGs, bloods, and physical health observations for shared care medication monitoring.
- Collaborate with the care home team to create dementia-friendly environments and provide meaningful activities for service users.
- First-level registered nurse (RNMH, RMN, or OT) with evidence of post-registration training in clinical/psychological interventions.
- Proven track record of clinical experience relevant to the post.
- Awareness and understanding of professional accountability, NMC or HCPC standards, and other local and national initiatives.
