Anticoagulant Clinical Specialist

We are seeking a highly skilled and experienced Anticoagulant Clinical Specialist to lead our outreach clinics at Woking Community Diagnostic Hub. As a key member of our team, you will be responsible for implementing treatment plans, managing patient caseloads, and contributing to the development of our anticoagulant service.
Main Responsibilities- Manage and lead outreach anticoagulation clinics at Woking Community Diagnostic Hub, ensuring high-quality patient care and efficient service delivery.
- Implement treatment plans for patients requiring anticoagulation therapy, working closely with healthcare professionals and stakeholders.
- Manage patient caseloads, maintaining accurate records and ensuring timely follow-up and education.
- Contribute to the development of our anticoagulant service, identifying opportunities for improvement and implementing changes to enhance patient outcomes.
- Collaborate with multidisciplinary teams, including GPs, SPA, and District Nurse Teams, to ensure seamless service delivery.

We are seeking a candidate with:
1st Level Registered Nurse or MPharm degree with integrated foundation training.Completed competencies for Anticoagulation Clinical Nurse Specialist, assessed by Consultant Hematologist.Experience within Anticoagulation at Specialist level.Experience of working within a multidisciplinary team.Customer service focus and willingness to take personal responsibility.-
