About the Role

We are seeking a highly skilled and experienced Anticoagulant Specialist to join our team at Ashford & St. Peter's Hospitals NHS Foundation Trust. As an Anticoagulant Specialist, you will play a crucial role in managing and leading on the outreach anticoagulation clinics at Woking Community Diagnostic Hub.

Key Responsibilities
  • Implement treatment of anticoagulation plans in the community
  • Manage and run the anticoagulant outreach clinics at Woking Community Diagnostic Hub
  • Liaise with GP, SPA, and District Nurse Teams
  • Provide initial warfarin education advice for patients, carers, and healthcare professionals
  • Maintain associate computerised records using DAWN and Cerner software
Requirements
  • 1st Level Registered Nurse or MPharm degree with integrated foundation training
  • Completed competencies for Anticoagulation Clinical Nurse Specialist assessed by Consultant Hematologist
  • Experience within Anticoagulation at Specialist level
  • Experience of working within a multidisciplinary team
