Key Responsibilities
  • Manage and lead on the outreach anticoagulation clinics at Woking Community Diagnostic Hub, ensuring seamless delivery of high-quality patient care.
  • Implement treatment plans for patients requiring anticoagulation therapy, working closely with healthcare professionals to ensure optimal patient outcomes.
  • Assist in the organisation and day-to-day running of the Anticoagulant Service, collaborating with colleagues to achieve shared goals and objectives.
  • Develop and propose service changes to enhance the department's performance, meeting the needs of the hospital and patients.
  • Manage a caseload of patients, maintaining accurate computerised records using DAWN and Cerner software.
  • Provide initial and ongoing education to patients, carers, and healthcare professionals on anticoagulation therapy and management.
  • Attend Trust Thrombosis committee meetings, contributing to policy decisions and initiatives related to anticoagulation therapy.
  • Ensure accurate and timely completion of documentation, adhering to Trust-approved guidelines and professional standards.
Person Specification
  • 1st Level Registered Nurse or MPharm degree with integrated foundation training.
  • Completed competencies for Anticoagulation Clinical Nurse Specialist assessed by Consultant Hematologist.
  • Experience within Anticoagulation at Specialist level.
  • Experience of working within a multidisciplinary team.
  • Desirable: Prescribers Course, Health-related degree, and evidence of further professional development.
