Job Summary

The successful applicant will be responsible for managing and leading the outreach anticoagulation clinics at Woking Community Diagnostic Hub. This will involve implementing treatment plans, assisting the team in the organisation and day-to-day running of the Anticoagulant Service, and managing a caseload of patients.

Main Duties

The successful applicant will be expected to:

  • Manage and run the anticoagulant outreach clinics at Woking Community Diagnostic Hub
  • Liaise with GP, SPA, and District Nurse Teams
  • Provide initial warfarin education advice for patients, carers, and healthcare professionals
  • Attend Trust Thrombosis committee meetings
  • Maintain anticoagulant software and be computer literate

Person Specification

The successful applicant will have:

  • 1st Level Registered Nurse or MPharm degree with integrated foundation training
  • Completed competencies for Anticoagulation Clinical Nurse Specialist assessed by Consultant Hematologist
  • Experience within Anticoagulation at Specialist level
  • Experience of working within a multidisciplinary team
