About the Role

We are seeking a highly motivated and experienced Social Care Professional to join our team in Buckinghamshire. This is an exciting opportunity for a passionate individual to make a real difference in the lives of adults in need of support.

Salary: £38,000 - £48,000 per annum, dependent on experience

The successful candidate will be responsible for delivering high-quality care and support to adults in their homes, promoting independence and enabling them to achieve their goals. As a Social Care Professional, you will work closely with other professionals and organisations to provide holistic support and ensure that individuals receive the best possible care.

Key Responsibilities:

  • To deliver high-quality care and support to adults in their homes, promoting independence and enabling them to achieve their goals.
  • To work closely with other professionals and organisations to provide holistic support and ensure that individuals receive the best possible care.
  • To develop and maintain strong relationships with service users, their families, and carers.
  • To identify and assess the needs of service users, developing and implementing effective care plans to meet their needs.
  • To work collaboratively with other teams and services to ensure seamless delivery of care and support.

Requirements:

  • A degree in Social Work or a related field.
  • Post-qualifying experience in adult social care, preferably in a leadership or management role.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, colleagues, and partners.
  • Able to work effectively in a fast-paced environment, prioritising tasks and managing multiple projects simultaneously.
  • Proficient in using technology, including electronic records systems and Microsoft Office applications.
