Responsibilities

Your key responsibilities will include:

  • Managing a caseload of patients within a community setting">
  • Providing specialist advice and support to healthcare professionals">
  • Acting as a resource to health care professionals within primary care, NI Hospice multidisciplinary teams, Trust multidisciplinary teams, agencies, and individuals">

Requirements

To be successful in this role, you will need:

  • Excellent clinical knowledge of specialist palliative care">
  • Effective communication skills">
  • Sound community awareness">
  • The ability to work independently and as part of a team">
