Patient Care Coordinator Role

We are seeking a highly motivated and organized Patient Care Coordinator to join our team at Chelsea and Westminster Hospital NHS Foundation Trust. The successful candidate will be responsible for providing exceptional patient care and administrative support to our clinical teams.
Key Responsibilities:- Receive and greet patients in a professional and friendly manner.
- Work in our Call Centre on a shift/rotational basis, answering calls from patients, booking appointments, and referring to clinical staff.
- Coding patients after attendance for income and public health statistics.
- Advising patients of current and approximate waiting times in liaison with nursing and medical teams.
- Updating patient DNAs and attendance using the Cerner and GUM EPR systems.
- Entering all new patient details accurately onto appropriate systems.
- Maintaining accurate patient demographic details and reporting any duplicate patient registrations to the Service Delivery Manager.
- Managing calls efficiently and professionally within identified performance standards.
- Noting any changes in patient GP contact details and addresses on EPR.
- Accurately attending all GUM patients as new, follow-up, reschedule, or rebook based on their last visit.
- Accurately processing all new HIV patients onto Cerner under a medical number.
- Effectively diffusing difficult situations with patients and referring to the Reception Supervisor or Nurse in Charge.
- Arranging and booking additional services required by patients, such as transport or interpreting services.
- Maintaining patient confidentiality at all times, keeping patient information private and only allowing access to authorized staff.
- Recording clinic encounters and RTT outcomes.
- Participating in audits of outpatient activity as required.
