Discharge Coordinator Job Description

Service Care Solutions is seeking a skilled Discharge Coordinator to join our team in Northampton. As a key member of our integrated multi-professional patient flow team, you will play a vital role in facilitating safe and timely discharges for patients on the ward.

Main Responsibilities:
  • Attend ward MDT meetings to discuss patient care plans and coordinate actions.
  • Participate in board rounds to ensure seamless communication and collaboration among healthcare professionals.
  • Complete discharge notifications for onward referrals to other discharge services, ensuring a smooth transition for patients.
  • Coordinate referrals to community nursing services and verify medication availability for discharge.
  • Arrange transportation and home assessment visits to ensure patients receive necessary support.
Requirements:
  • Foundation degree in a health-related subject or relevant experience working at this level.
  • Enhanced DBS clearance.
  • Desirable: experience working in a rehabilitation setting, preferably at an inpatient unit.
