Our Teams
- Community Hub Teams - Serving adults with varied needs from across Northamptonshire.
- Learning Disability Team - Supporting adults with learning disabilities throughout West Northamptonshire.
- Flow and Capacity Team (FACT) - Coordinating support for people in hospital, in reablement services, or transferring to care settings.
- Assurance and Safeguarding Team - Investigating cases of abuse or neglect, ensuring adults stay safely within their communities.
- Deprivation of Liberty Safeguards (DoLS) Team - Assessing individuals in hospital or care homes who may be deprived of liberty.
- Discharge and Review Team (DART) - Specialising in Continuing Healthcare (CHC) assessments, supporting people in residential and nursing homes.
What We're Looking For
We are seeking motivated individuals who are passionate about making a difference in adult social care. You must hold a recognised Social Work qualification and be registered with Social Work England. We welcome proactive, innovative problem-solvers with excellent communication skills who thrive in a collaborative environment.
