Job Title: Discharge Coordinator

Job Type: Contract

Location: Northamton

Salary: £22 Umbrella Per Day

About the Role:

The Discharge Coordinator will work in an integrated multi-professional patient flow team, facilitating safe and timely discharges for all patients on the ward. The post holder will coordinate timely patient referrals to relevant community services and liaise directly with patients and their relatives as required about discharge arrangements.

Key Responsibilities:
  • Attend ward MDT meetings to discuss patient discharge plans
  • Coordinate actions related to patient discharge, including referrals to other services
  • Complete discharge notifications and ensure medication is available for discharge
  • Arrange home assessment visits and progress meetings
  • Ensure all transport arrangements are in place
Requirements:
  • Foundation degree in a health-related subject or relevant experience working at this level
  • Experience working within a healthcare setting, preferably the NHS
  • Functional skills in literacy and numeracy to level 2 or equivalent
