About the Role

We are seeking a highly motivated and compassionate Rehabilitation Support Worker to join our team at The Disabilities Trust. As a Rehabilitation Support Worker, you will play a vital role in supporting individuals with acquired brain injuries to regain their independence and achieve their rehabilitation goals.

Key Responsibilities
  • Provide personalized support to individuals with acquired brain injuries, following individualized support plans to meet their needs and aspirations.
  • Develop and maintain strong relationships with service users, their families, and other healthcare professionals to ensure comprehensive care and support.
  • Work collaboratively with a multi-disciplinary team of occupational therapists, physiotherapists, psychologists, and speech and language therapists to deliver high-quality rehabilitation services.
  • Assist with daily living tasks, such as personal hygiene, meal preparation, exercise, and leisure activities, to promote independence and confidence.
  • Contribute to the development and implementation of individualized support plans, ensuring that they are tailored to meet the unique needs and goals of each service user.
Requirements
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, families, and healthcare professionals.
  • Ability to work collaboratively as part of a team, with a flexible and adaptable approach to meet the changing needs of service users.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Willingness to undergo training and development opportunities to enhance skills and knowledge in rehabilitation support work.
