Complex Healthcare Support Worker

We are seeking a highly skilled and compassionate Complex Healthcare Support Worker to join our team at City & County Healthcare Group. As a Complex Healthcare Support Worker, you will play a pivotal role in delivering expert care and support to individuals with complex needs, including those with spinal cord injuries, muscular dystrophy, and acquired brain injuries.
Key Responsibilities:
- Develop and implement personalized care plans in collaboration with healthcare professionals, prioritizing client-centered care and outcomes.
- Provide emotional support and companionship to clients and their families, promoting independence and well-being.
- Work effectively with multidisciplinary teams to ensure seamless care delivery and coordination.
Requirements:
- A minimum of 6 months' experience in a care setting, preferably with individuals with complex needs.
- Strong clinical knowledge and understanding of complex conditions, including spinal cord injuries, muscular dystrophy, and acquired brain injuries.
- Excellent communication and interpersonal skills to build strong relationships with clients, colleagues, and healthcare professionals.
