Community Health Nurse

We are seeking a highly skilled and dedicated Community Health Nurse to join our team at Calderdale and Huddersfield NHS Foundation Trust. As a Community Health Nurse, you will play a vital role in delivering high-quality patient care in the community, working closely with our district nursing teams to ensure that patients receive timely and effective care.
Main Responsibilities- Deliver direct and indirect patient care, including assessment, development, implementation, and evaluation of programmes of care.
- Participate in the creation of a professional nursing culture that is open to change, new ideas, and innovation, while reflecting organisational strategy and values.
- Work collaboratively with the team to maintain safe, effective, and responsive services, including flexible working arrangements as required.
- Registered Nurse level 1 or 2.
- An understanding of the healthcare needs of adults, especially older people.
- Individual assessment of health and social care needs.
- Hospital care experience.
- Adult care experience.
