About the Role

We are seeking an experienced Frailty Care Specialist to join our team at Sunderland GP Alliance. As a key member of our Frailty Team, you will work autonomously and collaboratively with Primary Care Networks to provide expert clinical care.

Key responsibilities include undertaking advanced clinical assessments, developing comprehensive care plans, and working collaboratively within multi-professional teams. Non-medical prescribing skills are essential for this role.

The successful candidate will possess a strong understanding of health promotion and counselling, as well as knowledge of quality improvement and research methodology. Experience of providing clinical supervision in practice is also highly desirable.

This is a challenging yet rewarding opportunity to make a real difference in the lives of patients. The estimated salary for this position is £50,000 - £70,000 per annum, depending on experience. Benefits include a competitive salary, 33 days annual leave, and a supportive work environment.
