Job Description:

As a Community Health Practitioner - Frailty Service at Sunderland GP Alliance, you will play a vital role in delivering high-quality patient care and contributing to the growth of our community-based services.

Main Responsibilities:

  • Work collaboratively with primary care networks and multi-professional teams to identify and address the needs of patients.
  • Conduct advanced clinical assessments and examinations to inform treatment decisions.
  • Develop and implement comprehensive care plans to optimize patient outcomes.

Requirements:

  • Registration with NMC/HCPC required.
  • MSc in an appropriate clinical field or relevant modules meeting the 4 pillars of advanced practice.
  • Dedicated time for continuous professional development.
