Care Coordinator

We are dedicated to delivering exceptional support and care to individuals with intellectual and developmental disabilities at Communitas, Inc. As a Care Coordinator, you will play a vital role in ensuring individuals receive holistic care and support in their homes and communities.
Estimated Salary: $17 per hour
Job Responsibilities:- Develop and implement personalized care plans that meet the unique needs of each individual.
- Collaborate with healthcare professionals, families, and community resources to ensure comprehensive support.
- Provide education and training on life skills, health management, and safety practices.
- Facilitate connections between individuals and community resources, promoting social inclusion and empowerment.
- High School diploma required; BA degree in Human Services or related field preferred.
- At least 1 year of experience supporting adults with developmental disabilities is required.
- A commitment to delivering high-quality care and support is essential.
- Valid Driver's License, reliable transportation, and acceptable driving record.
- Must pass company background and reference checks.
