Clinical Care Manager

Salary: £27,805 - £31,500 per annum (£14.11 per hour) plus 30% nights enhancement
Contract Type: Permanent
We are seeking a skilled and compassionate Registered Nurse to join our team as a Clinical Care Manager at Appoint Healthcare. As a key member of the inter-disciplinary team, you will be responsible for assessing, planning, implementing, and evaluating individualised care plans using evidence-based guidelines.
About the Role:
- You will work collaboratively with the care team to deliver high-quality patient care.
- Maintain accurate records in line with corporate policies and procedures, as well as NMC guidelines.
- Carry out all clinical care according to company policies and procedures.
- Ensure that patient care is provided in accordance with the inter-professional evidence-based care plan.
- When necessary, take charge of patient care during shifts.
