About the Role

As a Community Support Specialist at the British Red Cross, you will play a vital role in empowering individuals to overcome challenges and improve their overall well-being. Working closely with community-based health and social care providers, you will provide crucial support to those in need, helping them to develop independent lifestyles through connection with supportive groups and activities.

Your Key Responsibilities
  • Provide support and information to clients, aiding in the development of independent lifestyles.
  • Record and report client activities, ensuring appropriate actions are taken based on changes in their condition or circumstances.
  • Liaise with primary care, social care, health, housing, and other professionals to ensure comprehensive support for clients.
  • Collaborate with clients to establish short, medium, and long-term goals, developing personalised care and support plans.
