Community Health Nurse

We are seeking a highly motivated and experienced Community Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Community Nurse, you will be responsible for providing high-quality patient care in the community, working closely with patients, families, and healthcare professionals to deliver holistic care.
About the RoleThis is an exciting opportunity to work in a dynamic and supportive team, providing care to patients in their own homes, care homes, and clinics. As a Community Nurse, you will be responsible for assessing, planning, implementing, and evaluating patient care, using your clinical reasoning skills to develop individualized care plans.
Key Responsibilities- Assess patients' holistic needs, communicating complex and sensitive information to patients and carers
- Develop and implement individualized care plans, using evidence-based practice and clinical reasoning skills
- Work closely with patients, families, and healthcare professionals to deliver coordinated care
- Participate in multidisciplinary team meetings and contribute to the development of care plans
- Provide education and support to patients and carers on health and wellbeing
- Contribute to the development of policies and procedures related to community nursing
To be successful in this role, you will need:
- Diploma or degree in Nursing
- Current NMC registration
- ENB 998/mentorship preparation or equivalent
- Experience of working in a clinical setting
- Ability to provide high standards of care
- Highly motivated and able to work independently
- Excellent communication and interpersonal skills
- Ability to use own initiative and work flexibly
