Job Summary

We are seeking a highly motivated and experienced Community Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Community Nurse, you will be responsible for providing high-quality patient care in the community, working closely with patients, families, and healthcare professionals to deliver holistic and person-centered care.

Main Responsibilities
  • Assess, plan, implement, and evaluate patient care, using clinical reasoning skills to develop individualized care plans.
  • Conduct holistic assessments, communicating complex and sensitive information to patients and carers, and modifying care plans as needed.
  • Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care is coordinated.
  • Participate in multi-disciplinary/multi-agency meetings, contributing to the development of care plans and ensuring patient needs are met.
  • Ensure patient-held records are completed for each visit, and SystmOne inputting is completed on a daily basis.
  • Contribute to the requisitioning of supplies and equipment through electronic ordering.
  • Ensure informed consent is obtained prior to initiating treatment.
