About the Role

We are seeking a highly motivated and experienced Community Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Community Nurse, you will play a vital role in delivering high-quality healthcare services to patients in their own homes, care homes, and clinics.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care, developing individualized care plans that meet the unique needs of each patient.
  • Conduct holistic assessments, communicating complex information to patients and carers, and adapting explanations to ensure understanding and consent.
  • Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care is coordinated.
  • Participate in multidisciplinary meetings, contributing to the development of care plans and ensuring seamless service delivery.
  • Work collaboratively with the therapy team, including Physiotherapists, Occupational Therapists, and Assistant Practitioners, to provide a comprehensive service to patients.
Requirements
  • Diploma or 1st-level degree in Nursing, with current NMC registration.
  • ENB 998/mentorship preparation or equivalent.
  • Experience of working within a clinical setting, with a strong understanding of community nursing principles and practices.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
  • Ability to use own initiative, work flexibly, and prioritize tasks to meet service needs.
  • Basic IT and keyboard skills, with the ability to input data and maintain accurate patient records.
