Key Responsibilities- Assess, plan, implement, and evaluate patient care, developing individualized care plans that meet the unique needs of each patient.
- Conduct holistic assessments, communicating complex information to patients and carers, and adapting explanations to ensure understanding and consent.
- Liaise with GPs, Social Services, inpatient teams, and other healthcare professionals to ensure identified needs are met and care is coordinated.
- Participate in multidisciplinary meetings, contributing to the development of care plans and ensuring seamless service delivery.
- Work collaboratively with the therapy team, including Physiotherapists, Occupational Therapists, and Assistant Practitioners, to provide a comprehensive service to patients.
- Diploma or 1st-level degree in Nursing, with current NMC registration.
- ENB 998/mentorship preparation or equivalent.
- Experience of working within a clinical setting, with a strong understanding of community nursing principles and practices.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
- Ability to use own initiative, work flexibly, and prioritize tasks to meet service needs.
- Basic IT and keyboard skills, with the ability to input data and maintain accurate patient records.
