About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our leadership team at Right at Home Croydon to Caterham. As a key member of our team, you will be responsible for managing and owning our domiciliary care planning activity, ensuring that our clients receive high-quality care in the comfort of their own homes.

Key Responsibilities
  • Manage and own our domiciliary care planning activity, ensuring seamless delivery of care services to our clients.
  • Prepare and complete care schedules for our carers, utilizing Access People planner to optimize care delivery.
  • Build and develop care schedules with carers, reducing travel time and improving efficiency.
  • Onboard new clients to the schedule, matching them with suitable carers.
  • Develop professional working relationships with our care team and clients, ensuring exceptional customer service.
  • Provide first-line support for carers and clients with care-related inquiries.
  • Ensure compliance with regulatory requirements within the office and care calls.
  • Audit client records to maintain accurate and up-to-date information.
  • Support the registered manager in ensuring the smooth operation of the care service.
Requirements
  • 2 years of experience in care management, with a focus on domiciliary care.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, carers, and colleagues.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to work under pressure, with a flexible and adaptable approach to meet the needs of the care service.
  • Good computer literacy, with experience using care planning software and other relevant systems.
