We are seeking an experienced Community Learning Disability's Social Worker to join our team at LB of Greenwich.

Job Description:
  1. Conduct comprehensive assessments and develop individualised care plans that enable adults to achieve desired outcomes and maintain autonomy.
  2. Collaborate with adults, their families, carers, and other professionals to provide innovative, person-centred support that upholds individual rights.
  3. Facilitate access to advice, support, and services for adults and families, drawing on extensive knowledge of social care legislation, policies, and resources.
  4. Manage complex cases, making informed decisions, and taking proactive measures to support and protect adults and their families.
  5. Safeguard adults and children at risk by conducting thorough assessments and reviews in line with current legislation and best practice.
  6. Develop expertise in social care legislation, best practices, and local resources, contributing to a culture of learning within the organisation.
  7. Support the development of new initiatives, practice improvements, and policy enhancements within our organisation and in collaboration with external agencies.
  8. Maintain accurate and detailed records of activities, ensuring compliance with best practices and local guidelines.
Requirements:
  • A recognised social work qualification and active registration with the relevant regulatory body.
  • Proven experience in working with adults with learning disabilities and managing complex cases.
  • Strong knowledge of social care legislation, best practices, and current policies.
  • Dedication to upholding individual rights and promoting the wellbeing of adults with learning disabilities.
Salary and Benefits:
