St Paul's Primary Care Network is seeking an experienced Clinical Pharmacist to join our team in Cheltenham, Gloucestershire. As a Clinical Pharmacist, you will work across four practices within St Paul's Medical Centre, providing patient-centered care and optimizing medicines management processes.
Main Responsibilities- Work closely with practice leads to tailor the delivery of network priorities and targets with a practice-based focus.
- Undertake face-to-face and telephone-based patient consultations, reviewing patients with potential medication issues and long-term conditions.
- Offer professional leadership and facilitate shared learning with other clinical practice pharmacists and pharmacy technicians.
- Develop and maintain a good working relationship with community pharmacists, hospital pharmacists, and other local healthcare providers.
This is an exciting opportunity for a forward-thinking and innovative pharmacist to join our multidisciplinary team. As a Clinical Pharmacist, you will be expected to work collaboratively across the team, supporting less experienced pharmacists and technicians, and contributing to the development of the network.
Requirements- Enrolled in or have qualified from an accredited training pathway that equips the post holder to practice and prescribe safely and effectively in a primary care setting.
- Independent Prescriber or close to completion, with the ability to undertake face-to-face consultations with patients.
- Support and supervision from a Senior Clinical Pharmacist and GP lead to develop and do the job safely and confidently.
As a Clinical Pharmacist with St Paul's Primary Care Network, you will have the opportunity to work in a dynamic and supportive environment, contributing to the development of our network and making a positive impact on patient care.
