Job Summary

St Paul's Primary Care Network is seeking an experienced Clinical Pharmacist to join their team. As a Clinical Pharmacist in Primary Care Network, you will work across four practices in St Paul's Medical Centre, Cheltenham, providing patient-centred consultations and medication reviews.

Main Responsibilities

  • Undertake face-to-face and telephone-based patient consultations, reviewing patients with potential medication issues and long-term conditions.
  • Provide professional leadership and facilitate shared learning with other less experienced clinical practice pharmacists and pharmacy technicians.
  • Work closely with individual practice leads to tailor the delivery of network priorities and targets with a practice-based focus.

This is an exciting opportunity for a forward-thinking, innovative, and enthusiastic pharmacist to join our multidisciplinary team. As a Clinical Pharmacist in Primary Care Network, you will be expected to work collaboratively across the team and deliver the ethos of the network.

Requirements

  • Enrolled in or have qualified from an accredited training pathway that equips the post holder to practice and prescribe safely and effectively in a primary care setting.
  • Independent Prescriber or close to completion, able to undertake face-to-face consultations with patients.
  • Support and supervision from a Senior Clinical Pharmacist and GP lead to develop and do the job safely and confidently.
