Job Summary
St Pauls Primary Care Network invites applications for a part-time Clinical Pharmacist to join their team. The successful candidate will work across four practices located within St Pauls Medical Centre in Cheltenham, Gloucestershire.
As a Clinical Pharmacist, you will be part of the Pharmacy Team, working closely with GPs, existing Clinical Pharmacists, and the wider clinical team. Your role will involve providing a mix of face-to-face and virtual patient-centred consultations, as well as telephone support to patients and external agencies.
Key responsibilities will include prescribing within your competency, medication reviews, reconciling medications at hospital discharge, and clinical letters. You will also oversee the complex needs of housebound and care home patients, with the aim of reviewing and de-prescribing in polypharmacy.
About the Role
This position is ideally suited to an Independent Prescriber with experience in primary care. You will be expected to support less experienced pharmacists and technicians and encouraged to manage suitable long-term conditions. The successful candidate will work collaboratively within the clinical multidisciplinary team to advise on medicine optimisation, safety, and appropriate prescribing in line with local and national policy.

Key Responsibilities
- Work across the four St Pauls practices within St Pauls Medical Centre, Cheltenham, ensuring that medicines management processes are optimised to achieve highly effective, safe, and patient-centred prescribing.
- Work closely with individual practice leads to tailor the delivery of network priorities and targets with a practice-based focus.
- As an independent prescriber, undertake face-to-face and telephone-based patient consultations, reviewing patients with potential medication issues and long-term conditions.
- Offer professional leadership and facilitate shared learning with other less experienced clinical practice pharmacists and pharmacy technicians working in associated local practices and networks.
Person Specification
To be successful in this role, you will require:
- Enrolment or qualification in the Clinical Pharmacist training pathway: Primary Care Pharmacy Education Pathway (PCPEP).
- To be an Independent Prescriber or close to completion, with the ability to undertake face-to-face consultations with patients.
- To receive support and supervision from a Senior Clinical Pharmacist and GP lead.
- To plan and organise your own workload, including audit and project work, and training sessions for members of the medicines management team.
- To attend local, regional, and national meetings of relevance.
