About the Role:

We are seeking a highly skilled and compassionate Care Delivery Specialist to join our team at Nazareth Care Charitable Trust.

The ideal candidate will possess excellent interpersonal skills and be able to lead the care team in delivering a high standard of care to our residents.

Duties and Responsibilities:

  • Supervise and participate in the delivery of care to residents
  • Support and work closely with families and friends, offering professional and emotional support
  • Administer prescribed medication according to the home's policy and procedure
  • Lead, motivate, and support care staff
  • Be responsible for the preparation, implementation, review, and update of residents' care plans
  • Establish effective liaison with healthcare professionals

About You:

As a Care Delivery Specialist, you will be responsible for ensuring a safe, effective, and efficient care service is maintained, meeting the needs of our residents.

You will be expected to maintain clear and accurate records and communication systems relating to clinical and care delivery.

We are looking for someone who can work effectively as part of a team, supervise individuals, and model good clinical and care governance practice.
